INTRODUCTION

The Advanced Risk and Incident Analysis course is designed to equip professionals with the advanced tools and methodologies required to effectively identify, assess, and mitigate risks, as well as analyze incidents within various settings. In a complex and high-stakes environment like healthcare, understanding how to manage risks and respond to incidents is crucial for ensuring patient safety, maintaining regulatory compliance, and improving overall healthcare quality.

This course focuses on advanced risk management frameworks, including root cause analysis (RCA), failure mode and effects analysis (FMEA), and other incident investigation techniques. Participants will learn to critically evaluate risk factors, analyze adverse events, and design proactive strategies to prevent future occurrences. The course also emphasizes the importance of data-driven decision-making and the role of technology in identifying trends and managing risk in operations.

Day 3: Advanced Incident Analysis (Part 2)

  • Failure to understand the Process - Reactive Hazards Explosions
  • Deviations in Process Control - A serious Ethylene Oxide Explosion
  • Incident Re-occurrence – Organisations Have No Memory

Day 5: Advanced Risk and Incident Prevention Programme

  • Incident Analysis using a series of case studies:
  • So much went wrong - A major oil refinery fire and explosion
  • Emergency Response Analysis
  • Pro-Active Incident Prevention Measures
  • Pre & Post Risk and Incident Management
  • Key Points Summary

CERTIFICATION

  • Upon successful completion of this training, participants will be issued with Macskills Training and Development Institute Certificate
